Community Supported Care Model

Together with individuals with ID/A, their families, and other community stakeholders, WPHS is proposing a purpose built, innovative model to support individuals more efficiently with quality-driven, person-centered services. WPHS is committed to self-advocates and family members serving in leadership roles in the governance of the model.

Building on Governor Shapiro’s declaration that services for people with ID/A Intellectual Disability/Autism is a priority, WPHS is proposing a system redesign, the Community Supported Care Model (CSCM) that would provide annual federal funding necessary to sustain a multi year growth strategy by modifying the current managed care tax. This model would also more precisely define the state’s annual financial liability and can generate savings that could be reinvested in community supported services for people on the waiting list and enable providers to raise the wages of Direct Support Professionals (DSPs).

The CSCM will:

  • Ensure that each individual will have a single designated service navigator, supported by a multidisciplinary care team, to provide whole-person care including physical health, behavioral health,
ID/A home and community-based services, pharmacy, and long-term services and supports.
  • Address Social Determinants of Health such as housing, employment, and food insecurity.
  • Put the person, their family, and the providers they choose at the center of decision-making to ensure each person’s whole health.

The foundation of the Community Supported Care Model focusing on achievement of person-centered outcomes.

CSCM was developed in PA and is being considered by other states. CSCM utilizes a Prepaid Inpatient Health Plan (PIHP) regulatory framework to align payments and incentives, while leveraging federal benefits of a managed care model, all while retaining the local, community-based nature of the programs and maintaining the capabilities and resources of a larger managed care plan. Pennsylvania currently operates a successful PIHP: the Adult Community Autism Program (ACAP) in the Department of Human Services. This model is being considered nationally as the logical progression into ID/A managed care and the Commonwealth is in a position to embrace and stand out as a leader in this important space within the broader healthcare landscape.
